The Intermediate Care Facility for the Mentally Retarded (ICF/MR) is an intense level of service that is always provided in a residential setting. The facility is operated under stringent Federal regulations and standards that are monitored on a regular basis by a State Survey Team. The funding source for this service is Medicaid, therefore all candidates for admission must eligible for this program. Typically, the ICF/MR client has more significant learning, behavioral, physical, and/or medical problem s than the average developmentally disabled person. There is a constant system where the client's needs are assessed, a plan of habilitation and treatment is developed, needed professional services are arranged, and on-going training/treatment/monitoring is provided. Skill Creations, Inc. offers four basic models of ICF/MR services. The largest facility is a 10,000 square foot unit that houses 15 clients. The next smallest model is the 12 bed facility that is two separate 6 bed 3,000 square foot group homes located on adjacent properties. Another option is a single 6 bed group home. Our smallest option is a single 5 bed home. External day programs are offered at most of our ICF/MR homes.
